Notarial Forms under the Notaries Regulations
Regulation 9(b) of the Notaries Regulations, 5737-1977 provides that a notarial certificate is drawn up according to the forms in the schedule to the regulations. Here you will find all the forms in their binding versions, each in Hebrew, in Arabic and in a combined Hebrew and English version. Form 1: Signature Authentication
The basic notarial certificate: the notary certifies that the signer, whose identity was verified as required, signed the document in the notary's presence. Read more: Notarized Signature Authentication.

Form 2: Signature Authentication on Behalf of a Corporation or Another Person
For signing on behalf of a company or another person, together with proof of signing authority, such as a power of attorney or a board resolution.

Form 3: Signature Authentication of a Minor
For authenticating a minor's signature. The minor's age is stated in the certificate based on evidence presented to the notary.

Form 4: Signature Authentication Where the Notary Does Not Know the Language of the Document
When the document is written in a language the notary does not know, and the certificate is issued in accordance with the regulations, based on the signer's declaration or a translation presented to the notary.

Form 5: Signature Authentication Where the Signer Does Not Know the Language of the Document
When the signer does not understand the language of the document: the certificate is issued after the content was translated for the signer into a language they understand, and this is noted in the certificate.

Form 6: Certification of a Copy
Certification that the copy is identical to the original presented to the notary. Commonly required for diplomas, certificates and official documents submitted to authorities.

Form 7: Certification of the Correctness of a Translation
Certification that a translation is true to the original, where the notary is proficient in both languages. Read more: Notarized Translations.

Form 8: Receipt of an Affidavit or Declaration
For receiving an affidavit or declaration before a notary, after the deponent was cautioned to tell the truth.

Form 9: Receipt of a Translator's Affidavit
Where the notary is not proficient in one of the languages: the translator declares before the notary that the translation is faithful to the original, and the certificate confirms receipt of that declaration. Read more: Notarized Translations.

Form 10: Certificate of Life
Certification that a person appeared before the notary and is alive. Required mainly by pension funds and institutions abroad.

Form 11: Drawing Up a Document
Certification that the document was drawn up by the notary at the request of the parties.

Form 12: Authentication of a Prenuptial Property Agreement
Authentication of a property agreement made before marriage, under an authority parallel to that of the Family Court. Read more: Notarized Prenuptial Agreement.

Form 13: Certification of a Will
Certification of a will made before a notary, which has the standing of a will made before an authority under section 22 of the Succession Law. Read more: Notarized Will.

Form 14: Certification of the Cancellation of a Notarial Document

Physician's Certificate
The form prescribed in the schedule to the regulations for a physician's certificate, required when the person requesting the notarial act is hospitalized or bedridden.
